This patient presented with strong expressive lines when raising the brows and frowning, with a history of little to no improvement from prior Botox A treatments (both with past injectors and initially with me).
Because chart review demonstrated a history of underdosing in the past, we first retried Botox A at a full, appropriate dose to rule that out as the cause. After still seeing no response, we discussed likely resistance to botulinum toxin type A—which is rare (estimated around ~1% of patients)—and decided to try off-label Myobloc (botulinum toxin type B).
After treatment, you can see a clear softening of dynamic lines both at rest and with expression, while maintaining natural movement. ✨
Given the lack of standardized dosing protocols for Myobloc in this area, treatment required careful titration. A small touch-up was performed at follow-up to fine-tune the final result.
This was a first for me—and a great example of adapting when the expected response just isn’t there! We are both so happy with the result!

Botulinum toxin type A remains the gold standard for aesthetic wrinkle treatment. True immunologic resistance to botulinum toxin A is rare.
In most cases of perceived “non-response,” underlying factors such as dosing, injection technique, reconstitution, muscle strength, or treatment interval are more commonly responsible than true antibody-mediated resistance.
However, in carefully selected patients who demonstrate reduced or inadequate response after appropriate optimization, botulinum toxin type B (Myobloc) may be considered off-label as an alternative neuromodulator.
While it shares the same mechanism of action—blocking acetylcholine release at the neuromuscular junction—it is a distinct serotype that may offer clinical benefit in select cases.
