One of my favorite “hidden” Botox treatments is the platysma—the thin muscle that runs from your chest up to your jawline.

As we age, we naturally lose collagen and the skin becomes thinner, making the repeated contractions of this muscle more visible. Smiling, talking, and facial expressions can create noticeable platysmal bands and accordion-like lines across the neck.

By strategically relaxing the platysma with Botox, we can soften these bands and create a smoother transition from the jawline into the neck.

In this before and after, we treated the lower face and neck with Botox. Notice how much smoother her neck looks, with significantly less platysmal banding and a softer appearance with expression. The accordion lines that appeared when she smiled are also dramatically reduced, while maintaining natural movement.

The levator labii superioris alaeque nasi (LLSAN) often gets lumped into “gummy smile” treatment, but that’s only part of the picture.😁👄

In some patients, the real concern isn’t excessive gingival show, it’s that the upper lip just disappears with smiling. The LLSAN can be a strong medial elevator, so when it over-recruits, it pulls the central upper lip superiorly and slightly inward, reducing the amount of visible lip.

A small, targeted dose of neuromodulator here can soften that over-elevation. The goal isn’t to restrict expression, it’s simply about improving dynamic lip show in patients who feel like their upper lip “goes away” when they animate.

This is a subtle tweak, but it can make a noticeable difference in:
